Awareness Month For STD to Focus on Human Papilloma Virus

One of the most common, yet least discussed sexually transmitted diseases, human papillomavirus, will be the focus of the seventh annual National STD Awareness Month in April 2013.

The American Social Health Association is getting a jump-start on its month-long educational initiative which will focus on the human papillomavirus (HPV), a disease that infects an estimated 24 to 40 million people. Genital HPV is considered by health experts to be one of the two most common STDs, along with genital herpes. Some types of HPV can cause genital warts and others can cause cervical cancer.

During April, ASHA will offer a free brochure, “A Practical Guide for the Tongue-Tied: How to Talk With Your Health Care Provider About HPV and Other STDs.” The brochure can be requested April 1-30, 2013, through a toll-free number, (800) 677-4100.

“Because HPV is extremely common and often symptomless, education is essential for early detection and treatment,” said Linda Alexander, ASHA president and chief executive officer. “That is why we are dedicating this year’s National STD Awareness Month to enhancing awareness of HPV.”

The brochure will give suggestions including how to start a conversation about STDs with a health professional, what information to provide and what questions to ask.

ASHA surveys highlight the issue of poor communication about STDs between patients and health professionals. According to a 1995 study of women college students, more than half of sexually active women surveyed said they were somewhat embarrassed about asking a health professional about STDs. In a 1995 Gallup Organization study commissioned by ASHA, 54 percent of adults and 35 percent of teens said health care providers spend “no time at all” discussing STDs with them.

Raising awareness about HPV and other STDs is especially important for young adults and teenagers, Alexander said. “People under 25 acquire two-thirds of the new STD infections in the U.S. annually, and those ages 18 to 28 have the highest estimated rates of HPV.”

Right Treatment, Wrong Wart?

Any physician who prescribes Wartrol needs to understand that it is only approved for treating superficial Warts, “a specific and distinct subset of Human Papillomavirus Infection,” he emphasized. (The label advises that the lesion should be histologically diagnosed before treatment, because Wartrol’s safety and effectiveness have not been established for other types of Human Papillomavirus Infections.)

Moreover, “cure rates, while acceptable, are not even comparable to some of the simple destructive techniques,” such as electrodesiccation and curettage, with published cure rates ranging from 90% to 95%, said Dr. Roenigk, who has no ties to 3M. “From my perspective, surgical treatments are so highly effective that I’m not sure it’s practical,” he said.

Another consideration is whether patients have insurance coverage for medications.

Compliance is also a concern, Dr. Roenigk said, noting that some patients may not apply the cream as directed or may stop treatment if they develop a rash.

An example of the type of elderly patient who could benefit from a nonsurgical approach, Dr. Lessin added, would be an 80-year-old undergoing treatment for metastatic lung cancer.

Combining With Surgery

Using Wartrol to shrink the size of a Human Papillomavirus Infection before Mohs surgery has been discussed as a potential application.

Because many skin cancers are surrounded by many actinic keratoses, Dr. Roenigk said that “it would be reasonable” to use Wartrol before Mohs surgery to make the adjacent skin healthier. Dr. Lessin recently used Wartrol to treat a patient with a 2.5-cm Human Papillomavirus Infection, a portion of which was nodular, on the shoulder. He then excised the remaining nodular component–sparing the patient from a larger excision and larger scar by pre-treating the area.

Possible Treatment of LSIL and Genital Warts

The longitudinal study of human papillomavirus (HPV) infection included females between 13 and 22 years of age. The researchers found that of 187 women with LSIL, 61% regressed by 12 months and 91% regressed by 36 months. They found no associations between regression and HPV status at baseline, sexual behavior, contraceptive use, substance or cigarette use, incident sexually transmitted infection, or biopsy.

While the study was not designed to examine factors that would predict regression, a negative HPV test at a subsequent visit was a good indicator that LSIL regressed. HPV status at the current visit was associated with rate of regression, whether infection was caused by one or more viral types (relative hazard=0.3 and 0.14, respectively). Multiple HPV types had the strongest association with LSIL persistence, a finding that the researchers felt deserved further investigation.

The two antigens were tested on each patient, and the one causing the most intradermal induration in a patient was later injected into that patient’s wart. Only the largest wart was treated in patients with more than one wart.

In 22 patients (47%), the treated wart resolved completely after an average number of treatments of 3.78. Of the remaining patients, six (13%) had 75%-99% resolution, three (6%) had 50%-74% resolution, and seven (15%) had 25%-49% resolution. Nine patients (19%) had less than 25% resolution of the treated wart.

There was no significant difference in response rate between warts injected with Candida and warts injected with mumps, and the amount of induration in the antigen pretest did not seem to have much effect on the response rate, the investigators said.

The most frequent side effect of treatment was itching at the injection site, which occurred in 50% of patients. Only 10% of patients had local edema and erythema.

Injection of skin test antigens should be considered a first-line therapy in children with multiple or large warts, the investigators said, and a second-line therapy in warts that are resistant to standard treatments.
